It’s not BER. I’m wondering if something was trying to eat it. There’s one on the other side too. Just this one tomato. Any ideas?

I know what BER is, and have had a few tomatoes get it. There is zero black on this, and a corresponding hole on the other side. It started as a hole, and then formed this skin across it.

Well, I don't think it's Blossom End Rot either. I think it's the healed over damage from a hornworm or possibly a bird. It doesn't seem to favor sun scald. Here at the Muddy Bucket Farm, we generally cut out the damaged part and eat the rest (one way or another) when it gets ripe.

Ok, the picture does not show things very well shot from the front. I just now see that it is a hole, at first look it totally looked flat.
Yeah, probably a caterpillar kind of thing. Not sure if you have them there, but tomato moths are quite common around here, and their caterpillars make similar looking holes in the tomato (usually near the stem end however).

The critter you describe sounds like a hornworm. They eat the foliage and as much of the fruits (usually still green) as they can consume in their short lives. I've had two so far this year, and both were about 4 inches long and as big around as the end of my pinky finger. Here's a picture of a smaller one from my archives.

Sorry, but it looks like catfacing to me. No pest; just a fused blossom making a fused tomato with faults.
